This study aims to unravel the mystery surrounding the impact of embryo quality on the probability of ectopic pregnancies. The researchers also delve into the role of ovarian stimulation parameters in influencing the occurrence of ectopic pregnancies.

Material and Methods: Navigating Through the Cohort Study

The historical cohort study scrutinizes 15,006 clinical pregnancies resulting from non-donor IVF/ICSI, with 8,952 involving fresh embryo transfer and 6,054 involving frozen–thawed embryo transfer. The period under investigation spans from 2000 to 2017 in Finland. Notably, 61.4% of transfers involved a single embryo, and 38.6% involved double embryo transfers, all of which included no more than one top-quality embryo.

The research employs logistic regression to analyze various factors influencing ectopic pregnancy rates. These factors include the type of cycle (fresh vs. frozen embryo transfer), female age, number and quality of embryos transferred, presence of tubal factor infertility, and aspects related to ovarian response to gonadotropin stimulation.

Results: Shedding Light on Risk Factors

The study reveals that ectopic pregnancy occurred in 2.3% of cycles, with no significant difference between fresh and frozen embryo transfers. However, the quality of the transferred embryo played a crucial role. Ectopic pregnancy rates were lower (1.9%) in cycles involving top-quality embryo transfers compared to those with non-top quality embryos (2.7%). Tubal factor infertility was found to increase the risk of ectopic pregnancy.

The logistic regression analysis highlights lower odds for ectopic pregnancy after top-quality embryo transfers (odds ratio [OR] 0.72, 95% confidence interval [CI] 0.56–0.92, p = 0.007). Additionally, transferring two embryos, tubal factor infertility, and the quality of the transferred embryos significantly influenced the risk of ectopic pregnancy.
